Call your carrier and ask if you have coverage for nutritional counseling (CPT/Procedure codes: 97802 and 97803).

If you have coverage, you will need to find out if you can see a provider out of network and if there is a copay.

Out of Pocket fees:

- One hour assessment: $155.00

- Follow Up Appointments: $135.00/hr (can be split into two 30 minute sessions)

- PAYMENT REQUIRED PRIOR TO SESSION AND CAN BE MADE VIA VENMO OR PAYPAL
